For participating in the Anna University Zone-IV A Basketball Tournament to be held at Valliammai Engineering College, Kattankulathur, from 15^th to 16^th  September 2010.

Along with the Student Eligibility Performa the Director of Physical Education should bring the following documents of the Students Participating without fail:

a. Xerox copy of the 12th mark sheet.
b. Xerox copy of the 10th Mark Sheet & Diploma degree Certificate for diploma students.
c. Xerox copy of the 12th Mark Sheet, UG degree Certificate for PG students.
d. Xerox copy of the student’s previous semester Hall Ticket.

e. Lateral Entry Students must attach Fees receipt and Bonafide certificate.

 

Students who do not poses previous semester Hall ticket, can obtain a copy of the Duplicate Hall Ticket by sending letter from the respective Principal to Anna University Controller of Examination by Paying Rs.100/- .

Note:
The attestation in the student’s photograph should have both the student signature and also the Head of the Institution signature. 
Avoid facsimile of the signature in the form.

 

 

 

 

1. Students can obtain Duplicate Hall Ticket by sending letter from the respective Principal to Controller of Examination, Anna University, Chennai by Paying Rs.100/- .
2. Lateral Entry Students must attach Fees receipt and Bonafide certificate with the Eligibility Proforma.
